Healthcare premises cannot simply evacuate
In a building where occupants cannot self-evacuate, the strategy is usually progressive horizontal evacuation: move people to an adjoining compartment rather than outside. That depends entirely on compartmentation and on the alarm signalling the right zones in the right order. It is a cause and effect problem, and it is the part most likely to be wrong after a building has been altered.
